This is a highly autonomous role, ideal for a confident and experienced Registered Nurse who enjoys working independently, making clinical decisions, and building meaningful relationships with service users in the community. The role offers a diverse caseload and the opportunity to develop broad clinical knowledge across a range of community health needs.
   
About Our Role:  
  • Provide community-based nursing support to long-term service users in their homes
  • Conduct phone-based needs assessments and care reviews
  • Monitor, review, and support service user goals and progress
  • Ensure incidents and accidents are reported accurately and promptly
  • Deliver care and support across a range of clinical areas, including:
    • Medication management
    • Enteral feeding
    • Falls prevention
    • Pressure injury prevention
    • Wound care
  • Work autonomously while managing a diverse caseload
  • Understanding of ACC and Ministry of Health community contracts is advantageous
 
   Skills & Experience | He pūkenga tou
  • Minimum of five years' post-registration nursing experience in New Zealand
  • Community nursing experience is desirable
  • Current New Zealand Registered Nurse Annual Practising Certificate (APC)
  • Experience working with ACC is preferred
  • Full, clean New Zealand driver's licence
  • Strong communication, assessment, and clinical decision-making skil
